Diet iD x Epic™
Diet ID is now available via the Connection Hub on Epic™ Showroom. Health systems that use Epic™ as their EHR can now seamlessly collect diet quality data and other nutrition metrics directly into the patient chart.
Diet ID is a standalone integration. Patients access Diet ID via a simple web link, log into Diet ID with they MyChart™ login, take the assessment. Diet ID then posts data back to a flowsheet or custom vital sign in Epic™ so providers can see data in real time.
THE PATIENT AND PROVIDER WORKFLOW
Patients will log into your custom instance of Diet ID with their MyChart™ login (patient doesn’t need to complete Diet ID’s assessment from WITHIN MyChart™)
Once the assessment is completed, dietary analysis is generated in real time, and selected data points are posted to the patient chart
